ABSTRACT:Bactrim and a synthetic sulfonamide have been shown to be effective in the treatment of some common infections in HIV-infected patients who have an active and extensive immune system and are immunocompromised. We assessed whether these agents may be used in the treatment of selected infections in immunocompromised patients with HIV/AIDS. We assessed the efficacy of Bactrim and a synthetic sulfonamide in a single dose-titration clinical trial comparing it to an effective combination of sulfa and metronidazole.
METHODS:We performed a prospective, randomized, double-blind, placebo-controlled trial in HIV-infected patients with AIDS, treated with either Bactrim or a synthetic sulfonamide. Patients received either Bactrim or a combination of Bactrim with either metronidazole, a sulfa agent or sulfonamide monohydrate.
RESULTS:Patients received Bactrim (n = 22) or a combination of Bactrim and metronidazole (n = 20) for 7 days, and metronidazole (n = 20) for 7 days. Bactrim showed a significantly lower rate of cure (p = 0.001) and fewer treatment emergent adverse events (p = 0.003) when compared with sulfa, which is consistent with a previous report that sulfonamide therapy may be associated with fewer opportunistic infections in patients with AIDS. Patients with a previous history of severe skin disease or liver dysfunction should be included in the study.
CONCLUSION:Bactrim and a synthetic sulfonamide may be used in the treatment of HIV-infected patients with an active and extensive immune system and in combination with a sulfa agent.

A total of 47 patients were included in the study, including patients who had a known history of HIV infection, as well as patients with a previous history of severe skin disease or liver dysfunction. Patients were given either Bactrim or a combination of Bactrim with either metronidazole, a sulfa agent or sulfonamide monohydrate. After 7 days of treatment, patients were assessed for the efficacy of the combination of sulfa and metronidazole, the combination of Bactrim with metronidazole, the combination of Bactrim with metronidazole, and the combination of Bactrim with metronidazole plus the other two agents. The overall cure rate for patients with active and extensive immune system was significantly higher than the cure rate for patients with active and extensive immune system alone. In patients with severe skin disease or liver dysfunction, the overall cure rate for patients with active and extensive immune system alone was significantly higher than that of patients with active and extensive immune system alone. In patients with severe liver dysfunction, the overall cure rate for patients with active and extensive immune system alone was significantly higher than that of patients with active and extensive immune system alone. No patients had received any other therapy or received other therapies in the previous 12 months. In patients with severe kidney dysfunction, the overall cure rate for patients with active and extensive immune system alone was significantly higher than that of patients with active and extensive immune system alone. No patients had received any other therapy in the previous 12 months. There was no significant difference in the cure rate between patients with active and extensive immune system alone.
